* fix: prevent duplicate assistant messages in TUI (fixes#35278)
When startAssistant() is called multiple times with the same runId,
it was creating duplicate AssistantMessageComponent instances instead
of reusing the existing one. This caused messages to appear twice in
the terminal UI.
The fix checks if a component already exists for the runId before
creating a new one. If it exists, we update its text instead of
appending a duplicate component.
Test coverage includes verification that:
- Only one component is created when startAssistant is called twice
- The second text replaces the first
- Component count remains 1 (prevents regression)

Long URLs that exceed terminal width get broken across lines by pi-tui's
word wrapping, making them unclickable. Post-process rendered markdown
output to add OSC 8 terminal hyperlink sequences around URL fragments,
so each line fragment links to the full URL. Gracefully degrades on
terminals without OSC 8 support.

* fix(discord): defer component interactions to prevent timeout
Discord requires interaction responses within 3 seconds. Button clicks
were routed through the LLM pipeline before responding, exceeding this
window and showing 'This interaction failed' to users.
Now immediately defers the interaction, then processes the agent
response asynchronously.

highlightMatch() was replacing tokens inside ANSI escape codes,
corrupting sequences like [38;2;123;127;135m when searching for '2'.
Fix: apply highlighting to plain text before theme styling.
- Add regex caching to avoid creating new RegExp objects on each render
- Optimize smartFilter to use single array with tier-based scoring
- Replace non-existent fuzzyFilter import with local fuzzyFilterLower
- Reduces from 4 array allocations and 4 sorts to 1 array and 1 sort
Fixes pre-existing bug where fuzzyFilter was imported from pi-tui but not exported.
- Extract formatRelativeTime to shared utility for reuse across components
- Optimize FilterableSelectList with pre-lowercased searchTextLower field (avoids toLowerCase on every keystroke)
- Implement custom fuzzy matching with space-separated token support and word boundary scoring
- Use matchesKey utility for consistent keybinding handling (arrows, vim j/k, ctrl+p/n)
- Fix searchable-select-list to support vim keybindings consistently

Replace hardcoded escape sequence checks with the pi-tui keybindings API to ensure consistent cancel handling across different terminal configurations.

- Store maxVisible as instance property instead of bracket access
- Remove unused filterInput theme property
Users can now type to filter sessions in real-time:
- FilterableSelectList component wraps pi-tui's fuzzyFilter
- Matches against displayName, label, subject, sessionId
- j/k navigation, Enter selects, Escape clears filter then cancels
- Uses derivedTitle from previous commit for better display
